Kidnapped by the Joker as a teenager, but rescued by Batman, Susan Starling developed an horrified fascination with the world of costumed criminals. After finishing school she studied psychiatric nursing. She worked in several secure mental hospitals. At one point she was offered a job with Arkham Asylum, which she turned down, fearing to work where the Joker was held.

At the age of 24, she became a nurse at Hanover Institute for the Criminally Insane in California. There she provided treatment for some of the most terrifying female costumed criminals- Black Widow, Madam Tseng, Minerva, Six-Gun Sally.

Despite her admiration for Head Nurse Riva, Susan became disatisfied with Hanover. She came to feel that the treatments provided were ineffective and reliant on psychiatric ideas which she saw as fads. She became somewhat restless in her role as Hanover nurse.

During her time at Hanover, 'Blue Snowman' and Inventa two enemies of Wonder Woman were held as patients. She learned from them about Transformation Island, a penal colony established by the Amazons of Themyiscira. She learned about how the female inmates of that prison were reformed through love and discipline, as well as physial activity. Susan became fascinated by Transformation Island and desired to see it for herself.

Queen Hippolyta welcomed Susan to Themiscyra and her nursing skills were quickly employed on Transformation Island, as well as her physical skills in handling unruly inmates. Susan was deeply impressed by the Amazon prison and became convinced that it was far superior to Hanover Institute. However, she had concerns about the Amazon's reliance on the magic Venus Girdles to control prisoners.

Returning to the USA, Susan persuaded both the Federal government and millionaire Brad Coulson to provide funds for an experimental women's prison along the lines of Transformation Island. Having faced an epidemic of female costumed criminals, the government was willing to provide assistance to any initiative to deal with the menace of super-crime.

On a remote island in the Pacific, Susan established the Mala Correctional Facility for Female Offenders, better known as Mala Island. It was her decision to name the facility after Mala, warden of Transformation Island, an Amazon who rarely received the credit so often enjoyed by Wonder Woman.

Mala Island would combine both the modern psychiatric approach of Hanover Institute with the holistic and spiritual approach of Transformation Island. Lacking the Venus Girdles of Transformation Island, it's staff are more reliant on physical force to control prisoners, but also offer a range of physical activities lacking in the bleak confines of Hanover.

Many of Susan's former colleagues at Hanover have temporarily served on the staff at Mala Island. In particular, the former inmate and nurse, Jenny Bunker became the first permanent member of staff at the facility.
